Payment and Fees

We are a non-profit medical clinic providing services at a discounted price based on household income and size. This benefit is available to all patients that qualify, including both those with insurance and those without insurance. Our fee schedule is available for your review in office

 

For those who are interested in financial assistance for your care, please bring a government-issued ID and proof of income (Click here for a list of what you can bring as your proof of income.) 

 

Insurances
 

We accept Aetna, Anthem (Blue Cross/ Blue Shield of Georgia), Ambetter, Cigna, and Humana!  We are also a preferred provider organization of Multiplan.

 

We also accept most forms of Medicare (except United Healthcare Medicare). To confirm your Medicare plan is accepted, please call your insurance company.

 

We now accept all forms of Medicaid including Peach State Health Plan, Amerigroup, Caresource, and Wellcare.
